Ideal candidate skills and experience

• Current New Mexico RN nurse license
• Minimum of 3-5 years experience within Geriatrics with proven supervisory skills
• Basic typing skills along with a working knowledge of personal computing and word processing software are required.
• Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
• Ability to effectively communicate in English and understand and follow written and oral instructions.
• Must have a valid driver license and access to a private vehicle for business use.

RN CASE MANAGER ...will ensure residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change. Assists with supervision of care giving and coordination of health care needs within the community. Provide training, supervision, and assistance with community Health & Wellness Director.

Location:
RN Case Manager will support three communities in New Mexico including: Brookdale Place of Albuquerque, Brookdale Palce at Valencia and Ponce De Leon.
