Location: Albuquerque, New Mexico
Description: UNM Hospitals is currently seeking to employ LPN OUTPT I right now, this vacancy will be reside in New Mexico. Some weekends required. *
Provide nursing care and patient education to designated patients in ! an ambulatory setting. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Accountability:
PATIENT CENTERED MEDICAL HOME - Adhere to and promote the core expectations of the Patient Centered Medical Home
PATIENT CARE - Assist in patient needs assessment; take vital signs and medical history; assist in triage, admission, transfer and discharge processes; develop patient care goals
PATIENT CARE - Order and administer medications, excluding IV medications as assigned
PATIENT CARE - Perform treatments at the nursing level; assist medical staff with complicated procedures; follow up on diagnostic results
TRAINING - Orient new staff; participate in patient care conferences; participate in quality assurance activities
EMERGENCY MEASURES - Assist with or institute emergency measures for sudden, adver! se developments in patients
SUPPORT - Ensure maintena! nce of patient areas; assist in reception areas, as needed; ensure patient files include all diagnostic reports; document all patient care activities
PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"
Education Requirements:
LPN accredited program graduate
Licensur! e/Certification Requirements:
LPN Licensure current with the New Mexico Board of Nursing
CPR Certification for Healthcare Providers or for Professional Rescuers or must obtain within 30 calendar days of date of position
Experience Requirements - Preferred:
Bilingual English/Spanish
Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
1 year directly related experience
2 years directly related experience
Physical Demands Requirements:
Heavy Work: Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects or people. Physical Demand requirements are in excess of those for Medium Work.
Testing Requirements:
C-Net Medication Test
Tuberculin Skin Test required annually
