Post by Jobing - Job Title: Coord Patient Centered Med Hom Description: JOB SUMMARY: The Patientâ€"Centered Medical Home (PCMH) Care Coordinator ! (PCMHâ€"CC) will work as part of a multidisciplinary healthcare team in assigned primary care clinics, including but not limited to pediatrics or adult medicine. The PCMHâ€"CC will focus on coaching and coordinating care for designated highâ€"risk patients. The PCMHâ€"CC will work alongside the physicians and clinic staff in a PCMH, focusing on identifying the needs of complex patients while assisting the clinic to develop and improve processes for managing ABQ Health Partners (ABQ HP) patient population. This person will promote patientâ€"centered care, work side by side with providers and medical home team members, and actively participate in multidisciplinary patientâ€"centered team meetings.

The PCMHâ€"CC plays a key role in transforming ABQ HP Clinics into true Patientâ€"Centered Medical Home. Adheres to policies, procedures and regulations to ensure compliance and patient safety. Participation in Compliance and other important training is a condition of emplo! yment. ESSENTIAL FUNCTIONS: 1.Directly responsible for assessing, coordinating and integrating healthcare for high risk patients in the assigned PCMH clinics. 2.PCMHâ€"CC will work collaboratively with PCPs, staff and other health professionals to provide a medical home experience and care coordination across the health care continuum for all patients in PCMH.

3.Uses professional judgment and departmental guidelines to refer cases to physicians and providers. . 4.Ensures compliance with HIPAA and other applicable company policies and procedures as well as regulatory requirements. 5.Identify and address opportunities for quality improvement in all aspects of serving our customers.

As! sist in planning and implementation of systems changes and procedures t! o achieve overall organizational objectives. 6.Maintain effective communication with management regarding development within areas of assigned responsibilities and performs special projects as required. Qualifications: EDUCATION Minimum: Associates Degree in Nursing, Desired: Bachelor's degree in Nursing. Certified Case Manager EXPERIENCE Required: Three (3) to five (5) year of previous job related experience in a healthcare environment.

Preferred: At least 2 year experience providing case management and/or patient or care giver education (Diabetes Educator, Asthma educator, chronic disease management, ADD/ADHD, cardiac/congenital disorders) within health care environment. Electronic Medical Records (EMR) experience. Pediatric experience desired. KNOWLEDGE/SKILLS/ABILITIES PC proficient; data analysis skills; ability to communicate and interact professionally with coâ€"workers, management patients, and Providers; ability to motivate, counsel and consult with! patients, care givers; working knowledge of HMO's, PPO's, Medicare, Medicaid, and insurance plans; knowledge of CPT4/ICD 9 & 10/HCPCS codes.

LICENSURE Minimum: Valid NM LPN, RN preferred License or valid multiâ€"state compact license Current BLS Certification. Current driver's license and ability to travel to multiple clinics as needed.
